Home Hospice was founded in 1982, as the first hospice in our service area. After 42 years we are still the only non-profit hospice. We have become "Your Family's Hometown Hospice". Our commitment is to the care and support of a patient and their family dealing with an advanced, chronic or terminal illness. We help them address physical, emotional, psychosocial, financial or spiritual symptoms, needs and issues reflective of their disease process. We also provide a full range of bereavement support to their families. Which includes grief support groups, Camp Dragonfly a grief camp for kids, special family and community groups.

Our care and support is done through the services of an interdisciplinary team including physicians, nurses, social workers, aides, chaplains, volunteers and office staff who bring a high level of experience and expertise in patient care and bereavement services. Our team has never stopped in admitting or caring for people in need.

In our hospice and palliative care programs, we serve patients regardless of age, gender, race or financial status - our youngest patient was 1 day old and our oldest patient was 105 years young. We've served patients in their cars, living without water and electricity, in the woods, that move from motel to motel, those living in nursing facilities, country homes and estates. Wherever they call home is where our team provides services 24/7.

Needs

We are seeking funding to help support these special programs of our agency:

Our Charity Care Program includes hospice and palliative care for those without insurance providing the opportunity to serve those individuals who, because of financial status, do not qualify for Medicaid or are not eligible for Medicare. Home Hospice serves not only the patient, but also the caregivers which in most cases are family members. This program allows dying individuals and their families to experience hospice care and services without additional financial burdens. It is the final safety net for indigent individuals and their families in our healthcare system to have a quality end-of-life experience.

Camp Dragonfly is a grief and loss program for children 8 to 12 years of age who have experienced the loss of someone important in their life. Through the program we hold weekend camps, offer 1 on 1 counseling and provide additional workshops or materials to support children in the community. We continue to expand the program with Family Grief activities where the entire family, adults and children, come to work on grief and loss together. Camp Dragonfly is needed because it helps children 8-12 years of age develop life-long coping skills. It gives parents, families and guardians the opportunity to learn about the grief process and obtain skills that will help them in supporting the children's healing process.

The Pathways Palliative Supportive Care program (PSC) was created to provide individualized care to patients dealing with an advanced, chronic or serious illness, working in collaboration with physicians or other health care providers. As a community outreach service of Home Hospice of Grayson County, our goal is to help those deal with pain and symptom management. These patients are looking for help in finding balance with their health, emotional, spiritual and resource issues. The primary purpose of the Pathways PSC is to assist clients and families by supporting their wish to remain at home. We do this by helping them to understand choices and determine directions as they pursue treatment; to understand complicated medical circumstances; to be life enhancing through education, support and decision-making; and to provide support between hospital and doctor visits. To facilitate these goals the Pathways PSC Staff offers medical assessment and care, education, encouragement, spiritual support, advocacy and referrals to outside resources.

Our Center for Grief and Loss provides emotional and mental support to those dealing with loss. Whoever the person or persons, if they were significant to you, then their loss has an impact on your life. We provide 1-on-1 counseling for individuals and families and offer multiple support groups to help our clients find the best fit. Programs and materials are also important in providing examples or insight into our feelings. All programs are confidential and open to anyone in our community.
